A junior designer uses AI to generate a set of icons. They look fine at a glance. Clean lines, consistent style, modern aesthetic. They ship in the next release. Two weeks later, your accessibility audit flags that half of them fail contrast requirements. A customer reports that one icon is culturally offensive in their market. The design system team discovers the icons use a subtly different visual language than your established icon set.
